Cancellation, Missed and Tardy Policies
Cancellation and Missed Appoinments–
a. Cancellations can be made within 24 hours of appointments without any potential consequence. If cancellation is made with less than 24 hours of appointment, consideration will be given to each individual case and instance. If less than 24 hours, this may be considered a No Show appointment as explained in the next section.
b. Missed appointments– Missed appointments where no cancellation contact is received, is considered as a No Show. The first No Show will not incur a No Show Fee. After the second consecutive No Show, an $87 fee will be assessed to your account. Prior to making any additional appointments, the No Show fee will require payment. After the third consecutive No Show, you may be dismissed from the office.
c. Late Arrival– Late Arrival appointments may also be considered as a No Show appointment if over 25% of the appointment time. For instance, this would be 15 minutes late for an hour long hygiene appointment. If contact is made prior to appointment time, informing the office of tardiness, consideration will be made to maintain the appointment if possible. Otherwise, appointment may require rescheduling and still can be considered as a No Show.
d. Form Completion Time– Prior to being seated for your appointment, any required Health History, Updates or Insurance forms that require completion may delay appointment time and require rescheduling if insufficient time remains for being treated.
